Our Patient Portal allows you to get in touch with our office faster - eliminating time spent on the phone. Access the Portal on computers, smart phones, or tablets any time because at Integrity, your healthcare needs don't end at 5PM.

You Can Use the Portal for the Following:

  • Request Prescription Refills

  • Check Lab and Test Results

  • Review and Pay Billing Statements

  • Exchange Messages with Our Practice

  • Request Appointments

  • Non-Emergent Medical Questions

All patients can access the Portal 24/7, however prompt responses will be received during business hours.

How do I set up an account in the online Patient Portal?

  • Option One

    If you have called our office and scheduled an appointment, and you provided us with an email address, you will receive an email with instructions on how to activate your account. Let us know if you have not received your welcome email.
  • Option Two

    If you have not yet contacted us, but would like to set up your account, please open the patient portal and look for the "Pre-Register" link below the login form. Follow that link and set up your account as instructed. We will review and activate your account.
